What Matters: Caring Conversations about End of Life is Shomer Collective’s signature Advance Care Planning (ACP) program.
This evidence-based protocol is designed to help people think about their beliefs and values and understand how these inform health care decision-making.
What Matters is based on a conversation protocol developed by Respecting Choices®, and adapted for use in Jewish community settings.
This 7-hour online training will prepare you to conduct advance care planning conversations, including: exploring end-of-life medical decision-making, post-death considerations, and completion of Advance Directives, all while integrating Jewish wisdom into the process.

This training is designed for lay leaders or professionals working with adults in Jewish settings, and/or whose work includes end-of-life conversations.

Both days of the training sessions with Shomer Collective.
Four asynchronous online learning modules through Respecting Choices® (at your own pace). The Respecting Choices modules are included in your registration fees through Shomer Collective.

Continuing education credits for nurses and social workers are available, through Respecting Choices, for completion of the asynchronous online modules only. Continuing education credits are not available for the sessions with Shomer Collective.
